Pay Rate: £33.42 per hour

Job Description:

Liquid Personnel is recruiting experienced Social Worker for its client’s Older Adults Team at Waltham Forest.
You’ll be working with individuals aged 65+ with a range of needs, including physical frailty, dementia, and long-term conditions.

What will your responsibilities be?

  • Undertake strengths-based assessments and reviews
  • Develop and review care and support plans
  • Work closely with health professionals, families, and community services
  • Apply the Care Act 2014, Mental Capacity Act, and safeguarding procedures
  • Promote independence and person-centred outcomes
  • Use Mosaic / Liquidlogic (or relevant case management system)

Qualifications & Experience:

To be successful in this role you must have,

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
