Main duties of the job

The post is part of the Site Operations Centre that will be the ‘hub’ of the hospital and will promote safety and effective patient flow. As a vital member of our team you will be key in the development and the delivery of our site strategy.

The Band 7 post holder will support management of the site and operational oversight on a 24:7 basis and provide clinical leadership to support our acute patient cohort. You will also have responsibility for a portfolio of professional practice.

As a member of the team you will spend your time:
· Being part of the clinical response team providing clinical support to acutely unwell patients.
· Supporting the management and coordination of the hospital site on a day to day basis, including internal and major incidents.
· Coordinating patient flow through the emergency and elective pathways across the Trust.
· Being an integral member of the Hospital at Night Team.
· Providing senior support and leadership to patients, visitors and staff.

You must have excellent leadership skills and recent experience of caring for patients in an acute setting, as well as evidence of continuing professional development.

The successful candidate for this Band 7 post will be supported to obtain Advanced Life Support competency and complete an advanced assessment skills course as part of their professional development.

Detailed job description and main responsibilities

Key Relationships

All clinical areas, Inpatient Services Team, Junior Nursing staff, Ward Managers, Matrons, Lead Nurses, Heads of Nursing, Directors of Nursing, Hospital Director, Resuscitation Officers, Quality and Nursing staff, Medical Staff, Security Staff, Ancillary & Support Staff, External Agencies.

Job purpose:

  1. To triage adult inpatients in the acute hospital setting as part of the Hospital @ Night team (H@N) providing treatment under Management Pathways, independently as a Clinical Site Practitioner, and referring patients to the appropriate SCSP/ medical teams if required, and according to priority system.
  2. To provide an acute response for patients requiring step up or down from critical care areas.
  3. To assist the duty SCSP in providing senior nursing support and advice to all clinical areas within the Trust and provide leadership to the H@N teams.
  4. To participate in the Trust’s Emergency Response Teams to Fire, Security alerts, Cardiac Arrests and other clinical emergencies.
  5. To assist the duty SCSP in their responsibility ‘out of hours’ for the operational management of the Trust, implementing Policies and Procedures as required and providing support and advice to Trust employees.

Main Duties and Responsibilities:

Clinical:

  1. To provide senior nursing support, professional and clinical advice to all adult inpatient areas within the Trust
  2. To undertake advanced patient assessment, advising on or implementing immediate care or referral for SCSP/medical intervention ensuring the prompt and effective management of patients.
  3. To undertake a wide range of advanced clinical interventions, which includes Advanced Life Support Skills.
  4. To support staff in the management and care of patients requiring Level 2 / 3 type care and to provide an escort service for patients who require their level of care stepping up or being transferred from an area of higher acuity.
  5. To respond to and provide senior nursing support in the event of a clinical emergency or as a member of the Trust Resuscitation Team.
  6. To proactively seek out patients at risk of deterioration, performing board and ward rounds

Site and Duty Management

  1. To support a seamless service through the development of processes and communication with regards to bed management, emergency care performance and out of hours team.
  2. With the assistance of the SCSP, co-ordinate and support the Trust’s emergency response to Fire, Major Incidents, Security Alerts and Serious Untoward Incidents
  3. To play an active and supportive role within the Trust bed management plans, to advise on the effective utilisation of the Trust bed pool, during escalation and incident management.
  4. Initiate escalation through agreed protocol to SCSP or Silver manager out of hours
  5. To play an integral role in the operational development of the service.
  6. To support the Lead Nurse Clinical Site Management &H@N in achieving financial balance, and identifying where the service could make cost improvements.
  7. To ensure that clinical and other untoward incidents which includes failure to meet related targets as set out by trust and local policy are reported and escalated appropriately.
  8. Liaise effectively with all other disciplines within the Trust.
  9. To participate in the development of the Clinical Site Practitioner & H@N service by maintaining accurate records for the audit process and by liaising with clinical areas and other service users to evaluate the effectiveness of the service.
  10. To hold individual and collective responsibility for ensuring that the Clinical Site Practitioner and H@N Service operates effectively across the trust, participating where necessary in providing cover for colleagues’ unexpected sickness /absence.
  11. To contribute positively to the leadership of nursing within the Trust acting as an effective role model.
  12. To work with and support managers and clinical staff to identify areas where changes in practice are required.
  13. To introduce measures to actively manage and reduce risk to patients. Ensuring the formal reporting and recording of adverse incidents is in line with Trust policy.
  14. To support the SCSP in implementing Trust initiatives and strategies.
  15. Network with the other H@N Site Practitioner teams to evaluate, develop and promote the specialist-nursing role.
  16. To utilise and maintain information systems to aid audit and to provide regular actively analysis reports.
  17. Maintain contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity data as requested. To evaluate the service in terms of clinical effectiveness, clinical excellence and value for money.
  18. Ensure compliance with the Trusts’ Equality and Diversity Policy supporting the delivery of the Trusts’ Race Equality Scheme and the Trusts’ duty to positively promote race equality and equality of opportunity for disabled people ensuring services are responsive to the needs of equality groups.

  5. To attend meetings and conferences as appropriate ensuring that you are fully conversant with current issues with in both the Trust and within the specialist services locally and nationally.
  6. To promote and facilitate evidence based practice and clinical audit within the Trust, raising the profile and enhancing the contribution of nursing to patient care.
  7. To participate in and assist with any appropriate trust approved research projects.
  8. To promote nursing research and evidence based practice relevant to the speciality.
  9. To work with and support clinical staff to identify areas where changes in practice are required. Also where opportunities exist for improving treatment and support of patients.
  10. To publish and present relevant clinical practice and service developments

General:

  1. To undertake any other duties which may be appropriate to the post.
