An exciting opportunity has arisen for an experienced, innovative and committed Band 7 Clinical Nurse Specialist, based at our Denmark Hill site. This post is 6-month Maternity Leave cover.

The post holder will work autonomously helping to further develop nurse-led services alongside a wide multidisciplinary team whilst also supporting more junior members of the team.

The successful applicant will need to demonstrate high standards in clinical care, have sound management skills and have the ability to improve quality and patient satisfaction. You will be flexible, organised, resilient and able to meet the challenges and demands of a busy and diverse clinical service.

Significant clinical experience and expertise in HIV nursing and Sexual Health is necessary for this post.

Main duties of the job

  • To work as an advanced practitioner and effectively deliver an expert nursing service within the paediatric and adult HIV services.
  • To manage a caseload of patients both paediatric and adult.
  • To coordinate the smooth running of the family and adolescent clinics
  • To assist in the development of evidence based clinical nursing practice in line with current research and guidelines.
  • To establish clear lines of communication with the HIV team to ensure cohesive multidisciplinary management of patients
  • To link with healthcare teams in order to increase knowledge and facilitate the care of young people in the community
  • To take responsibility for identifying areas for audit and research; to carry our audit and research and to present and translate the application of research to practice development.
  • To provide advanced level, autonomous nursing care to patients with HIV – to include diagnosis and treatment of STI’s and related conditions. To utilise advanced knowledge and skills and undertake stable patient management, across sites, as well as supporting emergency clinics.

Detailed job description and main responsibilities

1. Clinical practice

· Work effectively as a specialist nurse for adults, children, young people and families living with or affected by HIV/AIDS .

· Provide long-term support for patients with HIV/AIDS.

· Establish a trusting relationship with the child/young person and his/her family.

· Assess if a child/young person is Fraser competent to give informed consent to investigation, treatment and sharing of information to a third party.

· Assess the nursing needs of the patient and family and to ensure appropriate equipment and support is available in the community for children and young people with continuing needs.

· To implement the guidance developed by BHIVA, BASHH and CHIVA for the HIV testing of children with HIV positive parents.

o To develop and follow clear procedures when testing children of HIV positive parents.

o To develop robust information systems to record and monitor the process of testing all the children of their parents.

o Develop an agreed multidisciplinary pathway for testing children that includes plans for all eventualities.

· Run nurse led clinics:

o Support and counsel families in issues of disclosure within and outside the family nucleus.

o Support and counsel families regarding the HIV screening of children.

o Perform HIV testing on children and adolescents.

o Inform parents/carers and older children/young people of the results of HIV and its implications.

o Provide specialist nursing management for children and young people living with HIV and receiving prophylactic and/or antiretroviral treatment.

o Perform ongoing assessment of the nursing, psychological and educational needs of the child, young person and family with HIV/AIDS.

o Monitor babies born to HIV positive mothers.

o Educate young people living with HIV on all aspects of HIV.

o Provide information and advice on other related health issues as appropriate incl. STIs, safe sex etc.

o Provide adherence support for families and young people.

· To work as required in an offsite clinic providing HIV care in a community setting

· To attend and coordinate Multi-Disciplinary Team (MDT) meetings and ensure they contribute to the delivery of a seamless service for patients.

· To liaise effectively with other members of the MDT.

· To ensure all care is given in accordance with agreed protocols and to assist in the development of these protocols.

·To review clinical notes of patients attending clinics, ensuring all assessments, diagnostic investigations and test results are up to date and in place to ensure a smooth outpatient process.

· To establish and monitor a system to follow up children and young people who fail to attend clinic appointments.

· To maintain clear reports, records and relevant databases on all patient activity.

· To liaise with local community & voluntary organisations and social services in order to develop existing services for clients, carers and health professionals.

· To actively involve service users in providing feedback of their experience of the current service and suggesting improvements.

· To participate in the development of appropriate Patient Group Directives

· To provide support to young inpatients on wards if required.

· To support the youth peer mentors who attend clinic to provide peer support for young people.

2. Education and Research

· To lead on recruitment to current research studies within the department.

· To ensure all patients are fully informed of the risks and benefits of proposed procedures/treatments, contributing to the process of securing informed consent.

· To provide data to statutory and other agencies as necessary

· To produce and provide all patients with relevant and accessible written information regarding their treatment in accordance with Trust standards.

· To supervise and participate in the delivery of programmes of education and training according to identified needs of clients, carers and others involved in their care.

· To work with other clinical staff to identify knowledge gaps in relation to specialist patients and ensure staff are appropriately educated.

· To contribute to the development of nursing practice within the team. Additionally, where appropriate, to act as a practice supervisor to Trust and honorary contract employees.

· To actively participate, and lead in educational events provided by the trust and other organisations.

· To attend meetings and conferences as appropriate ensuring that you are fully conversant with current issues both within the Trust and within the speciality area of HIV/AIDS locally and nationally.

· To promote and facilitate evidence based practice and clinical audit within the Trust, raising the profile and enhancing the contribution of nursing to patient care, informing business and service development plans.

· To participate in and assist with an appropriate /relevant Trust approved research projects conducted within the department.

· To promote nursing research and evidence base practice relevant to HIV/AIDS.

3. Management

· To support the recruitment, selection and induction of new nurses into the service.

· To act as a role model to and provide leadership within the HIV nursing team and ensure good communication between and across staff groups.

· To assist in the resolution of informal complaints locally and in the investigation of, and response to, formal complaints as required,

· To investigate accidents and incidents, identifying and implementing corrective action where necessary, in conjunction with the Modern Matron.

· To liaise effectively with all other disciplines within the Trust and at local level.

· To identify service improvement opportunities and participate in the management and implementation of changes to improve standards for the benefit of patients and staff.

· To actively contribute to any relevant initiatives within the Division and provide support to colleagues and Heads of Nursing.

· To comply with measures to actively manage and reduce risk to patients, ensuring formal reporting and recording of adverse incidents in line with trust policy.

· To utilise and maintain information systems to aid audit and provide regular activity analysis reports.

· To maintain contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity data as requested in order to evaluate the service in terms of clinical effectiveness, clinical excellence and value for money.

· To ensure the best use of available resources is used within agreed budget to provide cost effective service.

· To forge effective links within primary, secondary and tertiary care to ensure effective communication and seamless patient care.

4. Personal Professional Development

· To achieve a range of clinical competencies as required to fulfil the role.

· To keep up-to-date with current literature and research in the area of HIV/AIDS.

· To maintain your own personal and professional development in accordance with Revalidation requirements, attending mandatory study sessions as required.

· To undergo and actively participate in own performance appraisals.

· To keep up to date with NMC, relevant specialist Faculty and Trust guidelines and protocols.

· To adhere to the NMC Code of Professional conduct working within and accepting responsibility for maintaining agreed levels of competence.

· To be working towards higher degree or equivalent (if necessary).

Person specification

Knowledge & Experience

Essential criteria
  • An advanced practitioner either through formal qualification (degree) and/or extensive clinical experience in either Sexual Health and HIV services or Paediatric services.
  • Relevant, demonstrable experience of clinical working in the field of HIV and Sexual Health
  • An understanding of the needs of children and young people and an ability to positively engage and communicate with this client group.
