PURPOSE OF POSITION

As a Tenancy Sustainment Support Worker, you will provide trauma-informed, person-centred support to residents as they prepare to move on from supported accommodation and transition into independent living.

You will work alongside residents before they move into their new home, helping them prepare for the practical, financial and emotional aspects of maintaining a tenancy. Following move-on, you will provide community-based support to help residents settle into their accommodation, sustain their tenancy and achieve greater independence.

This role plays a vital part in preventing homelessness, reducing tenancy breakdown and supporting residents to build stable, fulfilling lives within their communities.

Tenancy Sustainment Support Workers at Your Place help residents develop the confidence, skills and resilience needed to successfully manage their homes, access community resources and achieve their longer-term goals.

The role will require regular travel across the community and occasional evening and weekend work in line with service needs.

RESPONSIBILITIES & ACCOUNTABILITIES

As a Tenancy Sustainment Support Worker, you will play a key role in helping residents move successfully from supported accommodation into independent living. You will work alongside residents before and after they move, providing practical, strengths-based and trauma-informed support that helps them build confidence, settle into their new home and sustain their tenancy.

In this role, you will:

  • Support residents to prepare for move-on, including understanding tenancy responsibilities, planning the move and setting up a new home.

  • Provide one-to-one support and home visits once residents move into the community.

  • Help residents manage rent, benefits, budgeting, utilities and household responsibilities.

  • Identify issues early and take practical steps to prevent tenancy breakdown or repeat homelessness.

  • Connect residents with local services, health and well-being support, employment, training, volunteering and community opportunities.

  • Work closely with internal teams, landlords, housing providers, local authorities and specialist agencies to coordinate support.

  • Promote resident independence, confidence, resilience and community connection.

  • Keep clear and accurate records, including case notes, support plans, risk assessments and outcome evidence.

  • Respond appropriately to safeguarding concerns, incidents and emerging risks.

  • Contribute to reflective practice, team learning and continuous service improvement.

Experience

  • Relevant experience may be gained through employment, volunteering or education, including supporting people affected by homelessness, housing insecurity or multiple disadvantage.

  • Experience supporting individuals to live independently and achieve positive housing outcomes.

  • Experience working in a trauma-informed and person-centred way with people who may have experienced homelessness, mental ill health, substance misuse, offending behaviour, domestic abuse or social isolation.

  • Experience completing assessments, support plans and risk assessments and reviewing progress against agreed goals.

  • Experience working collaboratively with external agencies and coordinating multi-agency support.

Skills & knowledge

  • Ability to build rapport and maintain professional boundaries while working with vulnerable adults.

  • Strong communication, advocacy and motivational skills.

  • Ability to manage a caseload, prioritise competing demands and work independently within a community setting.

  • Good IT skills including Microsoft Office and case management systems such as In-Form, Salesforce or Pyramid.

  • Knowledge of welfare benefits, tenancy sustainment, homelessness prevention and supported housing services.

  • Understanding of safeguarding, confidentiality, GDPR/data protection and health and safety.

  • Understanding of trauma-informed practice, strengths-based approaches and recovery-focused support.

  • Ability to travel across the borough and undertake home visits.

Desirable

  • Experience supporting residents during move-on from supported housing into independent accommodation.

  • Knowledge of Universal Credit, housing costs and tenancy-related support services.

  • Experience using In-Form.

  • Qualifications relevant to housing, homelessness, support work or social care.
