Join our client's dynamic Occupational Therapy Team in Ealing, where you'll work across a diverse community to deliver high-quality, cost-effective services that make a real difference.

Key Responsibilities:

* Manage a caseload of customers with varying levels of complexity and risk
* Conduct specialist assessments in homes to determine eligibility and risk
* Recommend and arrange equipment and adaptations to promote safe, independent living
* Design and specify cost-effective home adaptations, including scale drawings
* Prescribe wheelchairs and accessories in line with NHS guidance
* Provide emotional support to customers and carers during major environmental changes
* Complete re-housing assessments and compile detailed reports
* Carry out moving and handling risk assessments and provide training to carers and professionals

What We're Looking For:

* A qualified Occupational Therapist registered with HCPC
* Experience working with individuals with physical, mental, or learning disabilities
* Strong assessment and care planning skills
* Ability to work collaboratively with health, housing, and voluntary sector partners
* Commitment to safeguarding and promoting the welfare of vulnerable adults
