This post is a partnership role working across the Denmark Hill site of King’s College Hospital NHS Foundation Trust and community discharge services to support transfers of care for complex patients outside of the hospital bed base. The post holder will be responsible for the development and delivery of discharge services across the site, linking with Local Authorities and PLACE. The post holder will work in line with agreed objectives reflecting the targets, quality standards, resource controls and constraints, and will contribute to delivering the management strategy of the discharge service within the Trusts.
Main duties of the job
The successful candidate will be a skilled communicator and be able to communicate highly complex and sensitive information at all levels across multiple organisations, whilst always advocating for patients. The post holder will be a key stakeholder in the Trust’s revised Flow Improvement Programme and be conscious on how to implement this in practice whilst using the King’s Improvement Methodology (KIM) framework.
The post holder will be able to foster strong working relationships at senior levels with key strategic and delivery partners/stakeholders, across the NHS and wider health and social care sector. They will particularly excel at co-operation and collaboration in working across organisation boundaries. In line with these responsibilities, the post holder will oversee and ensure the development and coordination of strong delivery plans across the discharge/transfer of care services.
The Transfer of Care Lead will work with the Trust Senior Leadership Team and Senior Management Teams, external partners including our ICB’s, patient groups, third sector and Social Care providers, to ensure that the ambition is effectively translated into the local business plans. The lead role will ensure that the site has a clear and well-delivered plan to deliver improvements for long length of stay patients. They will have a good understanding of performance delivery metrics and that appropriate actions are being taken to maintain the scale and pace of improvement.

Detailed job description and main responsibilities
The role has three main delivery themes:
Strategic and Operational Development: lead the strategic and operational direction of the discharge/transfer of care services across KCH Denmark Hill site and effectively mainstream strategic priorities into commissioning and provider business across the health economy;
People management : support, lead, and develop high performing local delivery team to expedite swift discharges for our patients;
Partnership working : effective relationship management and engagement with senior clinical and non-clinical stakeholders; developing strong and credible network relationships with partner Organisations within KHP.
Key result areas:
Design and lead delivery of significant transformational change regarding how we manage transfers of care/discharge, to ensure we meet Trust, sector, and regional strategic and operational objectives, leading to measurable improvement in efficiency, safety and quality of care for patients.
Lead effective implementation of the programme plan with appropriate monitoring arrangements in place.
To implement a comprehensive programme dashboard effectively utilising existing data for the measurement of performance and modelling of outcomes, where possible working with IT colleagues in using electronic media to do so. There should be a strong reliance on the utilisation of EPIC systems to digitally support this improvement work.

Essential criteria
- Professional active registration (e.g. Nursing [NMC], HCPC, Social Care)
 - Higher professional or management degree master’s level (or evidence of active working towards this)
 - Evidence of relevant continuing professional development
 
Desirable criteria
- Post-management qualification
 
Knowledge and Experience
Essential criteria
- Recent experience of working at a senior level in transfers of care and optimising complex patient pathways across organisational boundaries
 - Experience of successfully managing significant organisational change
 - Ability to plan and organise a broad range of complex activities across different professional groups, which will require ongoing review
 - Experience of advising and influencing senior managers and clinicians in relation to risk management and quality improvement
 
Skills and Competencies
Essential criteria
- Demonstrable track record of improving care and services for patients
 - Understanding of current national and local health policy and application to medical / social services professionals
 - Excellent understanding of quality governance principles and delivery
 - Proven management skills demonstrated by successful resource management in a changing environment with a direct approach to problem solving, which reflects reality, requiring an effective and co-operative personal leadership style
