Outreach Team, support adults with a learning disability to maximise their independence within their own homes. experience of person centred planning, upskilling with cooking, budget and accessing the community.
flexible to work the following 4 week roiling rota:
weeks 1,2,3 and 4 – Tuesday, Wednesday, Thursday and Friday 3pm – 7pm
Sundays will be paid at an enhanced rate of pay
