Community Social Services Occupational Therapist
JustOT are recruiting for an HCPC registered experienced Occupational Therapist to work with our well-respected client based in London.

Hours & Days to be Worked: 35 hours per week
Essential Requirements : HCPC registered , 3 years Social Services Experience The post-holder will be part of a multi-disciplinary team (Social Workers, Occupational Therapists, and Physiotherapists) which provides a health and social care service to the local community. In general Occupational Therapists will:
- Be expected to care coordinate a caseload of people with varied complexity of needs.
Occupational Therapists at the higher grade will be expected to support those at the lower grade with complex cases as directed by the Team Manager.
- The Occupational Therapist plays a lead role in coordinating the care and support for users (and their carers/ families) pulling in the expertise of other professionals as required.
- They will also be responsible for accurate and comprehensive inputting of case management information for monitoring and planning purposes.
- The post-holder will aim to provide an efficient and cost effective health and social care service to service users, carers and their families, in accordance with agreed threshold criteria and using appropriate resources of Social Services, Health, the Council, the community and other non-statutory organisations
- To manage all clinical and professional aspects of complex casework, where appropriate, using evidence-based occupational therapy assessment, intervention, planning and reviewing/evaluation skills.
- To carry out all duties within the principles of Personalisation and enable people to retain their independence for as long as possible.
- To actively identify and maximise opportunities to re-enable service users to ensure that they continue as long as possible to live independent lives and maximise their choice and control.
- To manage cases effectively and respond to service users' needs in a timely manner within a highly pressured environment.
- Work in a collaborative way and seek advice of internal and external professionals e.g. OTs, Physiotherapists, District nurses, or other appropriate professionals to promote the wellbeing of the individual and ensure they have access to the required level of co-ordinated care.
- Offer advice and information regarding statutory and voluntary resources across the Authority to members of the multi-disciplinary team and other members of the social work team.
- Develop and promote the role of health and social work within the multi-disciplinary team.
- Where appropriate act as Care Coordinator to ensure a seamless service to individuals with complex needs and avoid unnecessary admissions to institutional care.
- Where appropriate refer on to other health, mental health or social care services e.g. Children and Families/District Nursing and work in partnership where this is required.
