An exciting opportunity has arisen for a motivated Registered Band 6 Haemodialysis Nurse to join our Home HD Team. We are seeking an individual with the flexibility, enthusiasm, and commitment to make a real difference to Haemodialysis patients who wish to have a greater control over their time and treatment by doing their dialysis within their home environment.

As a key member of the Home HD team, you will work with a small group of dedicated and enthusiastic staff who help make our patients desire for more flexible dialysis treatment become a reality. You will work with our two band 6 nurses to help provide supervision and leadership to the Band 5 and 4 in the team, ensuring the safe and effective delivery of the service to our patients. You will also be working closely with our renal consultants and other colleagues across Renal to ensure that current patients are well supported at all times and new patients are recruited to join the programme.

You will be expected to deputise for our Manager Band 7 when not available and support the Home HD team members as required.

We are looking for an inspirational renal nurse who looks to lead when required and has awareness of safeguarding processes. You will bring excellent interpersonal and communication skills, with the ability to motivate and support colleagues, while showing a strong commitment to teamwork, innovation and delivering the highest standards of patient care.

Main duties of the job

  • To provide autonomous nursing care to meet the needs of our renal home patients, including training, trouble shooting, assessment, investigation, diagnosis and treatment for the individual renal patient.
  • To work within the Home HD team, supporting with training, referrals, and managing patient training pathways whilst also being a point of call for advice for the rest of the team and outside renal practitioners searching for more information about Home HD educate their patients of that possibility.
  • Lead on patient recruitment for the team and provide professional and clinical leadership where required to the team.
  • To lead on the end of month blood results pathway for patients liaising with the MDT, to make any required changes to the patient’s treatment and feeding back to the patients and help them make the changes.
  • To support the role of the Senior Sister, other team members and the Renal Consultants in the implementation of the patient treatment plan.
  • To be actively involved and supportive of the Senior Sister, other Nursing staff in the development and implementation of programmes for the improving of Home HD Unit.

Detailed job description and main responsibilities

  • To contribute to the Home HD Unit within Renal services, demonstrating a supportive and flexible leadership approach and a proactive, responsible attitude so as to promote confidence in the team and the service.
  • To provide professional leadership and clinical supervision to the Band 5 and 4 in the team.
  • To provide nurse delivered renal care in accordance with current service protocol and competency framework.
  • To support the senior nurse to ensure the service specification for home patients is implemented correctly.
  • In conjunction with the rest of the team to contribute and provide robust training, procedures and processes for patients joining the Home HD programme.
  • To be responsible for leading in the unit when required.
  • To participate in scheduled meetings with MDT to ensure patient treatment management is robust.
  • To escalate result queries/issues to relevant parties including completing an in phase if errors occur.
  • In conjunction with the senior sister complete route cause analysis for any errors that arise and escalate appropriately to the relevant teams.
  • To communicate with the Senior Service Manager and Service Delivery Manager with results processes/pathways to ensure full understanding and clarity especially if changes/adaptations are needed.
  • In conjunction with the senior sister to review and analyse the result pathway and to manage obstacles to reduce risks, errors and ensure best patient care.
  • To monitor competencies and to update the Results administrator competency book and SOP when appropriate.
  • To facilitate clinician understanding of the results pathway and ensure this is carried out.
  • To review and analyse clinician test requests to ensure tests are requested correctly/ appropriately and to be proactive at teaching staff when assistance is needed. This may also include escalating if incorrect pathways continue.
  • To contribute to and provide robust procedures and processes for the provision of training and management of patients on the Home HD programme.
  • To support the senior nurse in exploring current practice, identifying areas for development, and contributing to service changes as required.

Education and Qualifications

Essential criteria
  • 1st level Registered General Nurse
  • Further professional studies relevant to the clinical area
  • Mentorship program or equivalent teaching qualification
  • Renal Course

Skills and Competencies

  • Communication skills – able to relate and communicate with patients and staff at all levels and from different professions. Adapts communication style as necessary and is able to work with and through others.
  • Team building and working skills – able to lead a team to achieve results

Knowledge and Experience

  • Demonstrable clinical expertise, relevant to the area.
