Are you ready to make a real difference in people’s lives? Our Hospital at Home (H@H) Service is looking for an Occupational Therapist to provide expert, home-based care that helps patients stay independent and recover safely at home.

Deliver assessments and interventions across care homes, residential units, and patients’ homes.

Work closely with a supportive multi-disciplinary team to reduce hospital admissions and facilitate early discharges.

Supervise and mentor junior staff and students.

Prescribe, order, and teach the safe use of equipment.

Build strong partnerships with colleagues, patients, families and local agencies.
