2 LEARNING DISABILITY SOCIAL WORKER – PLEASE DO NOT SUBMIT CANDIDATES WITH LESS THAN 5 YEARS SOCIAL WORK POST-QUALIFICATION EXPERIENCE

The candidates will be office-based at HASS Fairlop – 852 Cranbrook Road Barkingside IG6 1HZ – the working pattern hybrid – 2/3 days in the office and the rest from home + 5 days office-based when on Duty. The Umbrella/LTD hourly rate is between £30 to £32.

Community Health and Social Care Fairlop Cluster (HASS Fairlop) is an integrated health and social care service.
The Team works with people with varied needs, including physical disabilities, dementia, Parkinson’s, Autism, ME, MS, Head injuries, Spinal Cord Injuries, Learning disabilities, etc., from 18 years to the end of life. Practitioners’ caseloads will reflect the varied needs accordingly.
Fundamental to fulfilling the responsibilities of this post is the ability to work collaboratively and to respond flexibly, positively, and successfully to the ever-changing pressures that local authorities and their partners face.
The candidate must assist Redbridge Council in fulfilling its duties and responsibilities concerning the Care Act 2014, National Framework for NHS CHC assessments, Mental Capacity Act, Deprivation of Liberties, Safeguarding, and other appropriate legislation.
This Job Description guides the level and range of responsibilities the post holder will be expected to undertake initially. It is neither exhaustive nor inclusive and will be changed from time to time to meet changing circumstances and demands.

Key Accountabilities Specific to this Post:

  1. To provide face-to-face holistic and comprehensive strength-based case management services to all clients, including care management assessments/reviews and carer assessments/reviews, risk assessments, mental capacity assessments, best interest assessments and ordinary residence transfers using your ability to present ideas relating to individual needs to include a range of options.
  2. To manage and outcome complex Continuing Healthcare cases and disputes. To work collaboratively with Integrated Care Boards (ICBs) as guided by the National Framework for CHC.
  3. To liaise and consult with users, carers, colleagues, and various statutory and voluntary agencies to compile information to assess client needs.
  4. Ensure that users have access to translation and interpretation services and that Braille and signing facilities are available wherever possible.
  5. To be sensitive to the dynamics of the client’s social and cultural situation and facilitate communication – to offer sensitive social work collaboration and an appropriate level of counselling and advocacy.
  6. To facilitate client independence and empowerment as far as possible by ensuring that all stages of a client’s assessment are strength-based involving the client and carers. Ensure that clients and carers are fully informed of their rights and access all relevant information.
  7. To produce detailed assessment/care plans/reviews/reports as necessary and maintain up-to-date computerised client records of a high standard to ensure good social work practice. Additionally, to provide all required documentation to service users and their representatives on time.
  8. To work proactively and extensively in partnership with other agencies, including health, education, carers’ services, careers’ services, and other statutory and voluntary agencies, to facilitate joint services planning for service users and their families.
  9. To work closely and positively with clients and their families to identify their needs and goals and coordinate necessary resources to meet those goals in a safe, effective, efficient, equitable and person-centred manner, ensuring that individual care plans are responsive to all needs of users, including their religious, cultural and linguistic needs, and the needs of disabled service users.
  10. To work with the Commissioning Team and the Housing Resettlement Services. Extra Care Housing and Supported Living providers to identify non-residential options for service users where appropriate.
  11. To liaise with service users and their representatives to signpost/refer them to support services and resources, including benefits, advocacy, housing, citizen’s advice bureau and other external agencies which may assist in meeting their needs.
  12. To maintain a courteous, helpful and polite response to users at all times and to ensure that individual needs are recognised and supported.
  13. To attend multidisciplinary case conferences, meetings and reviews, as appropriate, identifying the individual needs of users, recommending appropriate packages of care, and ensuring that service users and carers are supported in making a full contribution to meetings.
  14. To work with the Transition Team to manage the smooth handover/transfer of cases to the adult team, ensuring that all entries are updated on Carefirst by the responsible person accordingly
  15. To participate in the Team’s Duty Rota, responding to calls requiring urgent intervention, including situations which require investigation and action under Safeguarding Procedures.
  16. To respond effectively and sympathetically to any complaints made about the provision of services following the appropriate Council complaints procedure.
  17. To work with the Systems Team and support it with managing its desktop and data entries, training will be updated accordingly to facilitate this.
  18. To maintain records using database and spreadsheet applications for record keeping ensuring an excellent social work practice, including the use of Carefirst and RiO to input and retrieve data and to produce reports.
  19. To ensure targets are met per Service work programmes and produce statistics relating to workload.
  20. To participate fully in developing and maintaining good liaison and cooperation with the local community, community groups, and other agencies and non-statutory organisations.
  21. To participate in regular supervision with a Senior Practitioner.
  22. To participate fully in the Local Authority’s employee communication system.
  23. To attend and participate positively in team meetings as required to promote the development of an integrated response to clients’ needs, concentrating on efficiency and effective service.
  24. To provide cover for the work of other team members in their absence as required.
  25. To keep updated with local policies and procedures and always follow them accurately.
  26. To perform all post functions consistent with legislation and within the local authority’s policies, plans, strategic aims, standards and guidelines.
  27. To act on all cases of harassment per agreed procedures. To advise and support users suffering harassment.
  28. To contribute to developing and maintaining the Team Resource and Information database.
  29. To be smart and presentable in compliance with the current dress code.
  30. To deliver any other duties consistent with the post level from time to time as required.
  31. Financial and Asset Responsibilities: 1. To work towards developing user-centred, outcome-focused, quality and cost-effective packages of care for individuals within defined budgetary limits.
  32. To present cases to the Adult Resource Allocation Meeting (RAM)for approval of the personal budgets Staff Responsibilities
  33. To support, mentor and supervise junior staff as a Senior Practitioner directs.
