Salary: £25,154 per annum
Sanctuary Personnel, a leading recruitment agency within the health and social care sector, is currently looking for an experienced Health and Wellbeing Practitioner to join a reputable service in Heathrow on a permanent, full-time basis.
In this role, you will work within an integrated healthcare framework, providing substance misuse recovery support to individuals within a secure setting. You’ll deliver a range of interventions that promote recovery, wellbeing, and resilience, while helping people make positive, lasting changes in their lives.
Key Responsibilities
- Carry out initial and comprehensive assessments, including risk management and recovery planning.
- Manage a caseload of service users and deliver both group and one-to-one interventions.
- Facilitate structured programmes focused on substance misuse, low-level mental health, and general wellbeing.
- Promote services and signpost individuals to relevant internal and external support networks.
- Work collaboratively with multidisciplinary teams including healthcare, probation, housing, and other specialist agencies.
- Maintain accurate and professional records in line with GDPR and data protection standards.
- Support discharge and release planning to ensure effective continuity of care.
- Contribute to ongoing service improvements and uphold quality and safety standards.
Requirements
Essential:
- Level 3 Diploma in Health & Social Care (or equivalent).
- Experience conducting assessments and implementing SMART care plans.
- Strong understanding of issues relating to substance misuse and/or low-level mental health.
- Excellent IT and record-keeping skills.
- Commitment to equality, diversity, and inclusion.
- Strong teamwork, communication, and problem-solving abilities.
Desirable:
- Experience delivering structured interventions or facilitating group work.
- Must have CTC (CAT A) Clearance
- Previous experience working in secure, detention, or prison settings.
- Knowledge of CBT, motivational interviewing, or other evidence-based approaches.
- Lived experience of recovery, previous offending, or homelessness (with appropriate time since recovery/release).
- Understanding of the Health and Justice agenda and national drug strategy.
