Occupational Therapist - Hospital Discharge Service

Liquid Personnel is seeking an experienced Occupational Therapist to join a dynamic, fast‑paced Integrated Discharge Service (IDS) within Islington Adult Social Services. You’ll work as part of a multidisciplinary team of OTs, PTs and social workers supporting residents returning home safely and independently from hospital.

What will your responsibilities be?

  • Screening referrals for the Discharge to Assess pathway (7‑day rota; approx. 2/5 days per week once inducted)
  • Post‑discharge assessments to deliver one‑off interventions, equipment, minor adaptations and manual handling recommendations
  • Strength‑based reablement planning to support independence, recovery and prevent readmission
  • Collaborative working with Reablement, Social Work teams, the Integrated Front Door and First Point of Contact services
  • Hybrid working available after induction (home‑based documentation + community visits)
