The Post Holder Will Work Collaboratively With The Service Lead, The Wider Diabetes Team And Other Members Of The Multi-disciplinary Diabetes Team To

Provide comprehensive, evidence based diabetes care for people with diabetes and healthcare professionals within the Bromley area.

Work as a highly skilled independent practitioner implementing and recommending treatment based on expert clinical judgement.

Provide comprehensive evidence based highly specialist clinical care, training and consultancy to both people with diabetes and other professionals. Providing advice, support and education to GP Practices, and providing care in a range of settings including the diabetes centre, patient’s homes, and nursing homes.

Contributes to the development of diabetes services across provider boundaries

Review all the patients currently being seen by the District Nursing service for insulin administration (2 or more visits per day). With newer insulins on the market, some of these patients may be able to reduce the number of insulin administrations they require per day.
