We are seeking a dedicated and experienced Locum Prisons Services Clinical Discharge Pharmacist to join our team in London. This role is crucial in ensuring the safe and effective use of medicines within the prison service, contributing to public health campaigns, and providing leadership in medicines management. What will your responsibilities be :
In this role, you will devise and implement SystmOne computer searches to identify patients at high risk of harm from medicines and those at risk of unplanned hospital admissions. You will provide leadership to ensure compliance with CQC standards, contribute to public health campaigns, and manage medicines data to support decision-making. Additionally, you will lead improvements in the discharge pathway, provide education and training to healthcare teams, and manage the implementation of changes resulting from MHRA alerts. Your clinical duties will include working with high-risk patients, promoting healthy lifestyles, and providing face-to-face clinics. You will also act as a responsible pharmacist in the dispensary, make clinical judgments, and communicate with external healthcare providers to ensure continuity of care.
Qualifications and Experiences :
- Registered Pharmacist with the General Pharmaceutical Council (GPHC)
- Experience in a clinical pharmacy setting, preferably within a prison or secure environment
- Strong knowledge of SystmOne and its applications in medicines management
- Proven leadership skills and experience in providing education and training
- Ability to analyze and interpret complex medicines data
- Excellent communication and interpersonal skills
- Experience in managing the implementation of MHRA alerts and product withdrawals
- Knowledge of NICE guidelines and their application in clinical practice
- Ability to work independently and as part of a multidisciplinary team
- Commitment to promoting public health and healthy lifestyles
