Key Responsibilities:
- Clinical Consultations: Conducting both telephone and face-to-face consultations with patients to diagnose and treat a variety of medical conditions.
- Prescribing medications and treatments as necessary.
- Providing advice on general health, lifestyle, and disease prevention.
- Chronic Disease Management: Managing patients with chronic conditions, such as diabetes, hypertension, and asthma.
- Implementing care plans and monitoring patients' progress.
- Health Promotion: Promoting health and well-being through patient education and preventive measures.
- Encouraging lifestyle changes to improve overall health.
- Emergency Care: Responding to urgent medical situations and emergencies.
- Collaborating with other healthcare professionals in emergency cases.
- Team Collaboration: Working closely with nurses, healthcare assistants, and other support staff to deliver coordinated patient care.
- Participating in multidisciplinary team meetings.
- Administrative Duties: Maintaining accurate and up-to-date patient records.
- Participating in administrative tasks related to the practice, such as meetings and training sessions.
- Professional Development: Engaging in continuous professional development (CPD) to stay updated on medical advancements and best practices.
- Participating in peer review and clinical audit activities.

- Well-established total triage model with a safe, efficient and equitable workload.
- Accredited training practice for GP registrars and fifth-year medical students.
- Excellent multidisciplinary support , including physiotherapists, clinical pharmacists, pharmacy technicians and social prescribers.
- Practice-based specialist clinical pharmacist leading diabetes care and long-term condition management.
- Dedicated Advanced Clinical Practitioner providing MSK assessment, musculoskeletal ultrasound and joint injections.
- Two GP partners, two salaried GPs and one paramedic.
- EMIS Web clinical system.
- Experienced administrative team with efficient workflows, minimising GP administrative workload.
- Teaching practice for GP registrars, medical students, physician associates and clinical pharmacists.
- Regular clinical and educational meetings.
- Consistently high QOF achievement.
- Home visits are rare, with routine visits undertaken by the Community Matron where appropriate.

- Full registration with the General Medical Council (GMC) and inclusion on the GP Register.
- Completion of relevant postgraduate training (e.g., GP Specialty Training).
- Membership of the Royal College of General Practitioners (MRCGP) is desirable.
- Excellent communication and interpersonal skills.
- Full-time or part-time employment.
- Collaborative work within a primary care team.
- On-call duties may be required.
