Required - Salaried GP for up to 6 sessions per week

Monday x1 session, Tuesday x2 sessions, Wednesday 2x session & Friday x1 sessions.

Job responsibilities
Job Summary
The post holder will provide a portfolio of general medical services; including managing a clinical team, managing a caseload or managing specific Groves Health wide project, ensuring the highest standards of care for all registered and temporary patients, ensuring clinical effectiveness and quality. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare working within the organisation. Ensuring that internal and external quality standards are met and maintained, including standards laid down by the Care Quality Commission and commissioners. Developing an effective team building, working relationships with clinical supervisors or mentors and other colleagues. Taking a lead role on CQC compliant projects, business and service development and maintaining a culture of continuous quality improvement.

Key Responsibilities:
Management of Patient Health & Illness

Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate.
To analyse and interpret history, presenting symptoms, physical findings and diagnostic information to develop appropriate differential diagnosis.
To prioritise health problems and intervene appropriately, including initiation of effective emergency care.
To formulate an action plan based on scientific rationale, evidence based standards of care and practice guidelines.
To use National, Community / Public Health assessment information and guidelines, when evaluating patient needs, initiating referrals, co-ordinating care and programme management.
To develop and utilise a follow up system within the practice to ensure patients receive appropriate services and evaluate same.
To provide counselling concerning drug regimes, side-effects and interactions and - if legally authorised - prescribe medication based on efficacy, safety and cost, from the appropriate formulary.
To order, perform and interpret common screening and diagnostic tests.
To communicate the health status of the patient to the relevant health care professionals.
To provide health promotion and disease prevention services to patients and their families.
To maintain accurate and contemporaneous records, utilising the practice's computer system and considering the Caldicott Principles in relation to all data handling.
Act upon most alerts (e.g. QOF) and reminder during a routine consultation.
Undertake surgeries during extended hours in accordance with the access needs of the practice population and the contractual obligation of the practice.
Provide cross over, where necessary, to other Groves Health practices.
Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate.
Provide preventative health care and advice.
Provide medical support as required to patients referred by nursing staff.
Provide free certification in line with Schedule 9 of NHS GMS Regulations (1992).
Adhere to the practice prescribing formulary, local and national guidelines.
Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Records clear and contemporaneous IT based consultation notes to agreed standards.
Collect data for audit purposes.
Compile and issue computer generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible.
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working in primary care.

Qualifications
Essential

A medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003
Not subject to suspension under section 41A of the Medical Act 1983
Qualified General Practitioner (completed certificate of Completion of Training CCT)
Currently on a PCT performers list and not suspended from that list or from the medical register
DBS clearance
Have had an annual NHS appraisal and evidence of professional development

Desirable

Evidence of further postgraduate educational activities in relevant fields
MRCGP
DCH
DRCOG
RCGP Substance Misuse Certificate Level 1 or 2
F2 Supervisor training/teaching diploma - this is essential for HDS
DFSRFH & competence in coil insertion
Evidence of CPD activities
Minor Surgery skills

Experience
Essential

Excellent communication (oral/written) skills.
Working knowledge of MS Office Applications.
Experience of practice based evidence based approach to medicine including patient evaluation and triage procedures.
Experience of working with Integrated Care Boards / Services (previously CCGs) or other stakeholders working directives.
Knowledge and deliver of the Quality Outcome Framework QOF.
Understanding of current guidelines in a GP Practice.
Experience and an interest in Clinical management, leadership or teaching.
Experience of developing and completing audits, analysing results / data.
Experience of implementing change and improvement in primary care.
Understanding of clinical governance and quality issues.
Experience of managing incident investigations and complaints.
Problem solving and a solution focused approach.
Good clinical skills and awareness of patient sensitivity.
Innovative and keen to explore new ways of delivery.
Knowledge of NHS.
Have an understanding of the needs of the vulnerable groups of patients that are registered.
