An opportunity has arisen for an experienced nurse to join the IBD Team at Kings College Hospital. This post is Maternity Cover for 12 months on a fixed term contract

The successful candidate will work alongside the Inflammatory Bowel Disease (IBD) Specialist Nurse Team to become an integral part of the established Nurse Led Service to provide support, advice and improved access for both inpatients and outpatients.

This is a full-time permanent position, which will suit a dynamic and motivated nurse with some experience in the management and care of IBD. It is an excellent opportunity to develop your role within an established IBD specialist nursing team. Training and supervision will be provided within the role from both nursing and medical supervisors, along with opportunities to attend relevant study as appropriate.

Main duties of the job

Responsibilities will include telephone helpline, telephone clinics, face to face clinical review, involvement in the planning and delivery all treatments for patients with IBD (this includes discussing treatment options, monitoring advanced therapies, advocating for patients & contributing to the multi-disciplinary meetings).

Detailed job description and main responsibilities

Clinical Practice

· To provide and manage an expert specialist nursing service for people with IBD

· To provide expert nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment

· To ensure continuity of a high standard of evidence based nursing care, assessing health, health related and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care. This can include:

· Managing a patient caseload

· Maintaining accurate records & updating databases.

· Ordering & reviewing diagnostic tests / results

· Making and receiving referrals

· Running clinics

· Maintaining the IBD Advice line (Telephone & Email)

· To be the liaison with the Colorectal team for the joint patients

· To be actively involved in in-patient care, liaising with ward staff to ensure high quality nursing care

· Where appropriate to deliver nurse led outpatient services for IBD patients

· To follow-up patients as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment

· To ensure all care is given in accordance with agreed protocols and guidelines

· To liaise with the ward staff to ensure appropriate and timely discharge/follow-up arrangement in place for patients

· Act as a patients advocate and counsel when informed discussion may lead choices being made concerning treatment options

· To work with the MDT to develop, implement and evaluate integrated care pathways and systems of MDT documentation

· To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

· To ensure safe drug treatment by taking and checking regular test results and react according to protocol

· Be aware of the mechanisms for identifying, reporting and managing clinical risk, and ensure that there are resulting action plans.

· Identify the specific educational needs of IBD patients and their carers so that they are empowered to manage their condition and seek appropriate help and advice.

  1. Communication

· Ensure clear communication using the recommended guidelines between all appropriate professionals involved in the patients care.

· Have the ability to use advanced communication skills to support patients and carers when significant news is given and throughout the patients’ journey.

· Work with the established guidelines and policies within Kings, with a view to the need to develop these guidelines and policies according to service need.

· Development and maintain links with all members of the IBD MDT to promote a seamless service and clear communication.

· Document clearly in the medical notes and ensure appropriate data collection is maintained by recording on appropriate data bases

  1. Education

· Utilise opportunities to develop skills and knowledge further, keep up to date with current practice in this field and attend accredited courses.

· Comply with mandatory training

· To be generally competent in computer skills

· Raise awareness among healthcare staff of the educational needs of patients, their families and carers, paying particular attention to public and personal attitudes to bowel disease and how they relate to political, social, economical and ethical issues.

  1. Research and Audit

· To maintain a high level of awareness of relevant research, issues and trends, and national recommendations within IBD care and in nursing generally.

· Participate in local and / or national research and audit, including the National Biologics Audit and Registry as appropriate

· To attend and participate in directorate audit activities

· Clinically evaluate research findings and facilitate evidence based practice in IBD

· Participate in collaborative research projects within the clinical setting that will improve the quality of care and health outcomes in IBD.

· To work alongside clinical and research staff to identify and organise potential clinical trials, which can be undertaken within the unit

· To assist in recruitment, registration, screening and follow-up of patients entering research trials

· To ensure that information is available for patients in writing, as well as given verbally to ensure that informed consent is obtained at all times

· To ensure that any adverse events are reported to medical staff, and to pharmaceutical company project officers

  1. Leadership

· Act as a leadership role model within the clinical setting and demonstrate expert knowledge and high standards of clinical practise.

· In conjunction with the lead nurse, assess own needs for regular clinical supervision and access as appropriate.

· Ensure that services are delivered effectively and efficiently within available resources

· Take responsibility for your professional development, updating knowledge to promote excellence in clinical practice as appropriate and monitoring via the trust appraisal system.

· Actively work to raise the profile of CNS’ within the trust.

  1. Professional

· Act in a safe and professional manner, maintaining and safeguarding patient confidentially at all times.

· Work autonomously acting as the patients advocate through the application of ethical, legal and professional knowledge and skills

· Participate in Directorate, Trust and National IBD initiatives.

· To be actively involved in the peer review process

· To maintain and develop professional competence within nursing and IBD care.

· To take responsibility for personal development and education with regard to PREP and IPR/PDP objectives

· Respect all patient’s cultural and religious beliefs at all times, working in conjunction with other healthcare professionals to understand and ensure culturally sensitive care ie religious leaders / health advocates.

· To act at all times within the sphere of the Nursing and Midwifery Council Code of Professional Conduct.

· Maintain a database to demonstrate individual patient costs within the service

· To keep up-to-date with current literature and research in nursing and the speciality

· To maintain your own personal and professional development in accordance with PREP requirements, attending mandatory study sessions as required

· To undergo and actively participate in your own performance appraisals

· To keep up to date with NMC, relevant specialist Faculty & Trust guidelines and protocols

· To adhere to the NMC Code of Professional conduct working within and accepting responsibility for maintaining agreed levels of competence

· To act as a role model

· To maintain a high level of awareness of relevant research, issues and trends within the specialist area of practice and in nursing in general

· To liaise with the London & South East IBD Nurse Nurses Forum and attend the external meetings.

· Attend external educational meetings after discussion with the Lead IBD Nurse

  1. General

· The post holder has a general duty of care for their own health, safety and well being and that of work colleagues, visitors and patients within the hospital, in addition to any specific risk management or clinical governance accountabilities associated with this post

· The post holder has an important responsibility for and contribution to make to infection control and must be familiar with the infection control and hygiene requirements of this role

Essential criteria
  • 5 years F/T Post Registration experience in relevant field e.g. IBD, endoscopy, gastroenterology, infusion, colorectal, relevant ward or other chronic disease area
  • Relevant post-registration qualification or a willingness to undertake this
