Job Title: Social Worker – Complex Care
Location: London Borough of Bexley
Rate: £32 per hour

The London Borough of Bexley is recruiting for a dedicated professional to join our team, specializing in assessment and planning, as well as case management. They are offering a pay rate of £32 per hour.

What will your responsibilities be?

In this role, you will conduct thorough assessments of service users’ needs, risks, and strengths, and develop and implement personalized care plans in collaboration with service users, their families, and other professionals. Additionally, the role involves managing a caseload of complex cases, ensuring timely and effective interventions, and regularly monitoring and reviewing care plans to adapt to changing needs.

Qualifications and Experiences:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Degree level or equivalent in Social Work
• Post-qualified experience
