Nurse Practitioner, PrEP in the Community Project

We’re seeking an experienced and motivated Nurse Practitioner to lead on the PrEP in the Community Project , improving access to HIV prevention across South East London. You’ll work closely with GPs and outreach services to deliver high-quality, inclusive care and provide expert clinical leadership. This is a full time role fixed for 9 months.
The ideal candidate will be an NMC-registered nurse with strong sexual health experience, excellent communication skills, and a commitment to health equity. A non-medical prescribing qualification, teaching and audit experience is desirable.

Main duties of the job
• In conjunction with the Clinical Lead for the PrEP in the Community Project, to be responsible for the clinical leadership of the project, demonstrating a supportive and flexible leadership approach and a proactive, responsive attitude as to promote confidence with partners in primary care and outreach service for the work.
• To act as a clinical role model in order to ensure that the primary care and outreach partners in the project are able to provide PrEP to people at risk of HIV acquisition in a way that is efficient and patient focussed.
• To support GPs and outreach workers across SE London in raising awareness of PrEP and the delivery of PrEP for people in primary care and outreach settings
• To promote and facilitate evidence based practice and clinical audit within the Trust, raising the profile and enhancing the contribution of nursing to patient care and informing business and service development plans.
• To work collaboratively with as other MDTs at Denmark Hill or Beckenham Beacon sites where care needs require
• To actively participate in education events provided by the trust and primary care sector.

Detailed job description and main responsibilities
Main Duties and Responsibilities
1. Clinical Responsibilities
· To provide health promotion, risk management/reduction, behavioural interventions and PrEP advice and provision for identified patients referred from primary care into the Sexual Health service.
· To take referrals from participating GPs across SE London as required to provide PrEP expertise and support to patients with identified vulnerabilities or increased risk of HIV acquisition.
· To maintain clinical credibility by acting as a clinical practitioner in own area of expertise, especially on PrEP but also including the provision of history taking, sexual health screening, diagnosis and treatment of STIs .
· To supply and administer medication as required via PGD or Independent prescribing as appropriate
· To develop and participate in specialist nurse delivered PrEP clinics as appropriate to the needs of the service
· To ensure that robust procedures for enhanced partner notification are implemented in practice in line with National Guidance where required.
· To ensure that there is a clear and consistent approach to results management across the PrEP in the Community project and directly contribute to the delivery of positive results as required
· To ensure that, where relevant, all patients are managed and processes are in place to support a consistent and effective active recall procedure for people at risk of STIs, HIV and unplanned pregnancy.
· To act as a nursing lead on PrEP and to ensure clinical staff are supported in PrEP provision and managing / referring complex cases
· To manage the pathway of patients referred for PrEP and to ensure that management, recall and behavioural interventions are robust
· To manage the pathway for any patients newly diagnosed with HIV to ensure rapid access to services, provide support as required and to ensure robust partner notification
· To be highly visible, accessible and approachable to colleagues, patients and the public to ensure that open and honest communication channels are created and sustained.
· To maintain an overview of the standards of care being delivered across the PrEP in the Community project and instigate remedial action if necessary.
· To ensure that patients follow agreed pathways of care and are supported in safe and effective outcomes
· To provide advice on PrEP issues as required both internally and externally,
· To work collaboratively and develop relationships with external agencies, particularly those working with marginalised and underserved grops to promote the service and facilitate patient referrals
· To plan, develop and implement education and outreach activities in line with agreed service priorities
2. Leadership
· To assist in the resolution of any conflicts within the project, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required
· To investigate accidents and incidents; identify and implement corrective action where necessary, in conjunction with the Clinical Lead.
· To lead on any specific service development projects as agreed with the Clinical Lead.
· To actively participate and support the use of Adverse Incident reporting within the service.
· In conjunction with the Clinical Lead, support the PrEP in the Community project to maximise resources ensuring that the project’s priorities are met and kept within the allocated budget.
· To support the team in the delivery and planning of evaluation of the PrEP in the Community project to improve patient care delivery.
· To act as a clinical role model, mentor and clinical supervisor for team members as required
· To represent the PrEP in the Community project an relevant meetings.
· To ensure compliance with the Trust's Equality and Diversity Policy, supporting the delivery of the Trusts Race Equality Scheme and the Trust's duty to positively promote race equality and equality of opportunity for disabled people ensuring services are responsive to the needs of equality groups.
3. Education and Training
· Within the team to lead and promote the development of the educational environment by actively contributing to the educational development of all qualified and unqualified staff.
· To work with the team in assessing the clinical skills required to deliver a safe and high quality PrEP service, identifying any deficits in this and developing programmes to address these issues.
· To deliver training and workforce development for GPs, outreach workers and members of the multidisciplinary sexual health and HIV team to improve knowledge and skills on the provision of PrEP.
· To deliver training to primary care, outreach workers and on external courses delivered by the department (e.g. STIF) as required and any ad-hoc training within the Trust.
· To proactively support the training and assessment of primary care and outreach partners in the PrEP in the Community Project.
· To act as a resource for junior staff. To guide and support developmental projects and advise on further resources inside and outside the Trust as appropriate.
· To lead and promote within the team the development of an evidence-based approach to PrEP provision.
· To promote, initiate and participate in appropriate nursing research.
· To ensure representation at local, national and international conferences.
Person specification
Education, Knowledge and Experience.
Essential criteria
- NMC Registered Nurse
- Extensive experience at Band 6 (or above) in Integrated Sexual Health
- An in depth knowledge of PrEP, STIs and HIV prevention
