Key Responsibilities
Your role as a Care Coordinator would include:
- Ensure compliance with all office systems and procedures.
- Ensure rota is allocated, maintained and assigned to appropriate staff
- Updating of care plans and providing support to the Care Teams
- Conduct Care staff supervisions and assist the other Care office staff with aspects of their role.
- On call duties
- Develop strong relationships with the customers, their families, and other healthcare professionals.
- Maintain accurate and up-to-date records of care provided and any changes in customer’s needs.
- Good communication and listening skills
- problem solving ability and a drive to succeed.
Requirements
It’s important to us that all Caremark Care Coordinators have the following qualities:
- Experience of domiciliary Care, Care Co-ordination, and a working knowledge of the care industry.
- Previous experience with Access Care Planning and People Planner desired.
- U.K. Driving Licence
- Excellent communication skills and the ability to build rapport with staff, clients and their families.
- NVQ level 2 or higher in Health & Social Care
