The Children's Critical Care are looking for a WellChild Complex Care Nurse & Discharge Co-ordinator to join the team. The WellChild post provides a unique opportunity to provide compassionate expert nursing advice and support to children with long term complex needs and their families. The post is centres on the needs of families and children in the children’s critical care department facilitating their journey from hospital to home.

Main duties of the job

Key components of the role incorporate facilitating pathways for children in line with national standards, such as the NSF for children with continuing care needs. You will liaise with community teams and partner organisations to provide a comprehensive high quality seamless discharge from hospital to home. In addition you will be the named healthcare professional for ongoing support. Having a child in critical care is extremely stressful for families and the positive impact of this role and the support you can provide cannot be underestimated.
This is a recognised role across the UK supported by the WellChild Charity. As such you will have ongoing support from the WellChild Charity including a national network of other WellChild nurses. You will have the opportunity to participate in WellChild Charity activities, including case presentations, publications, fundraising and annual meetings.
You will need to be a registered children’s nurse with a mentorship qualification and evidence of demonstrating leadership with the ability to operate autonomously.

Detailed job description and main responsibilities

Provide a specialist nursing service for a caseload of children in Children’s Critical Care with complex needs, ensuring expert nursing advice and support is given to patients, their families and other healthcare professionals following diagnosis, through treatment and timely discharge
Implement the Integrated Care Pathway for complex patients to ensure an accurate and appropriate plan of care
Be the named point of contact for the families and carers, ensuring they are fully consulted and involved in the treatment plan
Support and empower patients, their families and other carers to influence and participate in decisions concerning their care by providing information on a range of specialist nursing care and services
Provide expert knowledge and clinical competence ensuring care delivery is of a high standard and evidence based
Liaise with community teams to develop ongoing appropriate packages of care
Provide home visits to ensure a safe and appropriate environment is confirmed prior to discharge
Accompany the patient and family home on the day of discharge
Provide telephone support and home visits for families, offering specialist advice, support and education
Assess and manage critical and clinical events to ensure safe and effective care.
Maintain up to date and comprehensive (contemporaneous) records of care

Person specification

Education, Qualifications and Experience

Essential criteria
  • NMC registration
  • Teaching/mentorship Qualification
  • RN 1 (Child)
  • A degree in a health related subject

Desirable criteria
  • Higher degree in a health related subject
  • Experience in running research and audit programmes
  • Evidence of service Development
